Provider Registration

Our provider partners are at the heart of our operation here at the Foundation. If you are an audiologist practicing in Oregon, we would love to work with you!

Complete this form to register to become a partner audiologist. The information we request is collected for hearing aid request purposes and to ensure our audiologist partners are in good standing to practice.

Once registered, you will be able to use the Request Hearing Aids form to make device requests for specific patients. Please allow up to five business days for registration confirmation. If you have any questions, please email us at contact@frinkfoundation.org.

Guidelines for Partner Audiologists

To ensure consistent and quality care for all the patients who are fit with Frink Foundation hearing aids, we ask our providers to abide by the following partner guidelines.

  1. Patients must meet the requirements set out on the Who Qualifies page of the Foundation website. It is the responsibility of the partner audiologist to confirm eligibility.

  2. Partner audiologists shall not charge patients for any devices provided through the Frink Foundation or the fitting services associated with those devices. The $25 processing fee (if qualified) for the devices may be passed on to the patient at the partner audiologist’s discretion.

  3. Partner audiologists shall include at least 90 days of follow-up services at no charge. Following the first 90 days, audiologists may charge patients a reasonable fee for clinical services.

  4. Additional costs are at the discretion of the audiologist. Not to exceed their usual customary charges.

  5. The partner audiologist is responsible for maintaining all patient records under state laws and HIPAA requirements. Partner audiologists shall not share any patient information with the Frink Foundation.

  6. Partner audiologists shall not discriminate in the delivery of professional services based on language, dialect, race, color, ethnicity, religion, national origin, ancestry, age, sex, gender, gender identity and/or expression, sexual orientation, disability, military, veteran status, or any other protected classification recognized by applicable law, under Oregon law. Partner audiologists shall not collect patient nationality information unless otherwise required by law or by a payor.

  7. Partner audiologists shall abide by all legal and ethical guidelines laid out by their state licensure laws and federal rules.
